Checklist Items#
Checklist items is the question bank. Each question is created once and can be used by any checklist, and its answer history is collected across every audit that asked it.
The list#
| Column | Content |
|---|---|
| Question | The question text, with its help text below. |
| Type | The answer type. |
| Management system | The management system it belongs to. |
| Org units | The units it is scoped to. |
Filter the list by answer type using the chips, and by management system or org unit using the dropdowns. Filtering by an org unit matches its whole subtree. Select Clear filters to reset.
Select a row to open the question's detail page.
Answer types#
| Type | Auditor input |
|---|---|
| Yes / No | Selects one of three chips. The stored values are YES, NO and NA. |
| Score | Enters a whole number from 0 to 10. |
| Text | Enters free text. |
| Number | Enters a number. |
| Single choice | Selects one of the options you define. |
| Photo | Captures one or more photos, with an optional caption. |
| Timestamp | Selects a button to record the moment. The value comes from the server clock. |
Yes / No label styles#
For a Yes / No question, select a Label style to change the three chip labels. The stored values remain YES, NO and NA, so grading is unaffected.
| Style | Chips |
|---|---|
| Yes / No | Yes, No, N/A |
| Pass / Fail | Pass, Fail, N/A |
| OK / NOK | OK, NOK, N/A |
| i.O. / n.i.O. | i.O., n.i.O., n/a |
| Conforming / Non-conforming | Conforming, Non-conforming, N/A |
Single choice options#
Select + Add option and enter a label per option. Labels are multi-language. Select ✕ to remove an option.
Whether an option counts as a deviation is set by the non-conformance rules below, not on the option itself.
Non-conformance rules#
A rule states that a given answer is a non-conformance of a given grade.
Select + Add rule and complete:
| Field | Options |
|---|---|
| When the answer | is (exact match) or is below (numeric threshold). |
| This answer / This score | For is, the platform offers only answers this question can produce. For is below, enter the threshold. |
| Then it is a | A grade from the management system's severity scale. |
| Finding required | When selected, the audit cannot be submitted until the finding is described. When cleared, the auditor is asked for a finding and may decline. |
The consequence of the selected grade is shown below the grade field, read from the severity scale.
Rule order#
Rules are checked from top to bottom, and the first rule that matches sets the grade. Later rules are not evaluated. A question with no rules never produces a non-conformance.
The editor marks a rule as shadowed when an earlier rule already matches every answer it would, and names the rule that shadows it.
A single question can carry several rules with different grades, so one answer can be a minor deviation and another a major one.
A scale is required
If the question's management system has no severity scale, a rule has no grades to select. Bind the management system to a standard, or give it its own scale. See Management systems.
Reference photos#
Select the photo tile under Reference photos to upload example images showing the expected condition.
The auditor sees them read-only under a Reference heading while answering that question, and can select one to enlarge it.
Up to 8 reference photos per question.
Help text#
Optional guidance shown beneath the question during an audit. Multi-language, see Multi-language content.
Scope#
| Field | Effect |
|---|---|
| Management system | The domain the question belongs to. Determines which checklists can use it and which severity scale its rules grade against. |
| Org units | The units the question applies to. Used to scope automatic checklist assembly and question drawing. |
Org unit scoping is hierarchical. A question scoped to a unit is also offered for units above and below it in the same branch.
Answer history#
The lower half of a saved question's detail page shows how it has been answered.
| Section | Content |
|---|---|
| Usage | How many times the question has been answered across audits. |
| Answer distribution | A bar per answer value with its count. |
| Answer over time | A row per answer with the date, site, answer, conformance result and a link to any finding it raised. |
Create a question from the checklist designer#
In the checklist designer, select New question next to a section's question picker to create a bank item and add it to that section. Enter the question text and the answer type. Configure the rest on the question's detail page.
Delete a question#
Select Delete on the row or on the detail page. The question moves to the Trash.
Notes and limits#
- Rules match one exact answer, or a value below a threshold. Ranges, upper bounds, several values in one rule and conditions spanning two questions are not supported.
- Score answers are fixed at 0 to 10, and a question can carry up to 8 reference photos.
- The clause cannot be entered here. A question can carry a clause that findings inherit and the audit runner shows, but there is currently no field to set it.
